SPECIAL INSTRUCTIONS:
1.	Register early to assure the accommodation of your choice. You must make your own Hotel Reservation. USE REGISTRATION CODE # S04OPFI 5th District GROUP RATE # In either case, we still need you to complete this form and send a copy with a copy of our District Registration Form.
2.	List the names of all occupants of rooms. If two people are sharing a room, list both occupants and bracket their names together.
3.	Do not double book. Send in only one form for your hotel reservation if you are sharing a room.
4.	Do not be a NO-SHOW. If your plans should happen to change, CANCEL your reservation.
